| 内容記述 | The purposes of this study are to develop outcome monitoring system to routinely assess the degree of program goal achievement, and also to construct the monitoring system that makes possible collaborations between practitioners and researchers sharing the results of monitoring. Focusing on case studies of the system development efforts of monitoring in the research for vocational transition support program and the hospital discharge promotion program, we obtained suggestions with the process, the results and the presented assignments. As a main result the following heads is important when constructing the system: (1) Initiate simply; (2) Adapt to the needs of practitioners; (3) Strengthen dialogue and cooperation with researchers and practitioners; (4) Clarify the definition. Future, we will put more emphasis on trust-based system of cooperation between practitioners and researchers, and there is a need to build a practical and useful system along the flow of daily work of practitioners. |

| 内容記述 | モニタリング結果を実践家と研究者が共有できるシステムの構築を目指し、実践家と協働で、プログラムゴールの達成程度を日常的に評価・把握できるアウトカムモニタリングシステムを開発することを目的とした。就労移行支援プログラム研究と退院促進支援プログラム研究におけるシステム開発の取り組みを事例として、そのプロセス、結果を整理し、課題を提示した。 システム構築の際には、（1）シンプルに始める、（2）実践家のニーズに合わせる、（3）実践家と研究者との対話および連携の強化、（4）定義の明確化が重要である。今後は実践家と研究者との信頼関係に基づく協働体制をより重視し、そのためにも実践家の日常業務の流れに沿った、実践的で役に立つシステムの構築を目指していく必要性を明らかにした。 |
